Conduct in-home health assessments and create personalized care plans.
Administer medications, treatments, and wound care as prescribed.
Educate patients and families on managing conditions and promoting independence.
Monitor patient progress and adjust care plans as needed.
Supervise LPNs and Home Health Aides to ensure high-quality care.
Participate in interdisciplinary meetings to coordinate comprehensive patient care.
Active RN license in Arizona.
Minimum of one year of RN experience ( home health, hospice, or critical care preferred ).
CPR certification and a valid driver’s license with reliable transportation.
Strong communication, organizational, and interpersonal skills .
